PLEASE NOTE: 

• Everyone is required to become a member through the Online Member Portal.

• Please go to this website to start the process of becoming a member. On this website there is a video for New Members and a video for Renewing Members.  I highly suggest watching these videos before you jump right in!

  4. Concussion Form

  5. Concussion Certificate Instructions

  6. NMAA Physical Form - All Members must have a new physical completed after April 1, 2024

  7. Eligibility Form

• Your physical must be done April 1, 2024 or later.

• Concussion Form and Concussion Certificate are two different items. You need to complete both. 

• YOUR MEMBERSHIP WILL NOT BE PROCESSED UNLESS ALL PAPERWORK IS COMPLETED AS STATED AND PAYMENT HAS BEEN RECEIVED.
